:load failure clean up loaded files
When `:load`-ing a file that doesn't result in a successful outcome, the "loaded" file is removed again from the list of loaded files. This avoids having it be used as part of `:reload` which will probably not result in a positive outcome. Instead, the user is expected to retry `:load` before issuing a `:reload`. Fixes #50.
